Outstanding Alumni Chapter Programming Awards Application

You are invited to submit a nomination (or nominations) for the Outstanding Alumni Chapter Community Service, Professional, or Social Program Award for the current school year. The awards will be presented to the programs selected as most outstanding among P.A.D.’s alumni chapters. Winners will be announced and honored over the summer.

If you believe your chapter’s program(s) should be considered for these awards, please complete this nomination form. You may submit up to three programs for each programming category. Each program must be submitted on its own application.
