Officer: New Member Materials Request

Please complete this form ONLY if you are a P.A.D. chapter officer intending to distribute the P.A.D. membership certificates and pins to your new chapter members.

Your chapter must have a minimum of three new members to send the materials free of charge. If the request is received in a timely fashion will do our best to send materials in time for the first ceremony date and then monthly after that until the end of the semester.

Expedited shipping is not currently offered for membership materials, but it is not required that you have member certificates and pins to hold your initiation ceremony. If officers do not have initiation materials in advance of the ceremony, we suggest that you hand out a red carnation, which is P.A.D.’s official flower, in place of the materials. Please follow the prescribed rituals when setting up and completing your ceremony.
